Hi Steve,
My surgeon was not local. I'm in Michigan and I went to Dr. Lingeman for the HOLEP on Dec. 12, 2014. I had another problem earlier tonight. My whole body started turning red and itching. I'm sure it's an allergic reaction to the Ampicillin I started two days ago, so, apparently I am allergic to penicillin drugs. I took some Benadryl to try and stop the itching but I had to stop the Ampicillin and tomorrow or Monday I will have to call my local urologist in Ann Arbor to decide what antibiotic to now try in an attempt to cure the UTI. Unfortunately, the results of the urine culture I had showed that Ampicillin was the antibiotic that would be best for the type of infection I had. Now, I have to hope there is something I can take for the infection that will work. I found out a year and a half ago that I was also allergic to sulfa drugs including Bactrim. Thanks again for your help.

Hi Jack. I'm a bit confused. I thought it was Dr. Lingerman who did your surgery but you said your "local urologist" was "familiar with Holep". Obviously then he doesn't do the surgery. Have you contacted Dr. Lingerman yet? (I hope he's accessible). As I said before I consider myself very lucky to have Dr. Das here in Philly where he is not only my surgeon but also my urologist. So I was comfortable with follow up.
I hope you get answer tomorrow but I'd also recommend that you call Dr. Lingerman tomorrow and see what he says. After all, he did the surgery. Keep us posted.

Hi Steve,
Yes, my surgeon was Dr. Lingeman and he trained Dr. Joshua Leese who actually joined the local urology practice where I normally go to in Ann Arbor last fall. Dr. Leese has done quite a few HOLEPs both while training as a resident under Dr. Lingeman and since he has joined the Ann Arbor practice. It is Sunday evening and I still have "Red man's syndrome" where my body is fairly red along with the itching that comes with this allergic reaction to the antibiotic. I purposely did not call either office this weekend as they probably would had told me to go to the hospital due to the allergic reaction and I wanted to wait until Monday when the urologists are normally working. I'm not having any life-threatening symptoms from what I can see. Dr. Lingeman's office offered to put an order in to have a urine culture taken or I could go and have Dr. Leese do it and I chose the latter since if I have to see a doctor he is local while Indianapolis is 300 miles. I need a new antibiotic since I stopped the Ampicillin. I hope the UTI wasn't due to a stricture as I have read that a stricture can be very difficult to treat. Thanks very much, Steve for the concern. I'll make the call first thing tomorrow morning.

Actually, my allergic reaction to Ampicillin got somewhat worse. My kidney functions [BUN and Creatinine] skyrocketed and I was worried about permanent kidney damage. Fortunately, repeat blood tests showed that these levels on Friday were much better so I was a little relieved. I'm on a different antibiotic, Macrobid, which I finished last night. I'm pretty weak but still optimistic. One other thing that happened 2-3 days was that I started having incontinence without any warning. After sitting in my bed for 60 seconds I was surprised when standing up to find a puddle underneath me. This happened a few weeks ago, also. I'm not sure why but I guess with HOLEP that is something we just have to contend with happening. I'm just three and a half months out from surgery, though. Thanks for the thoughts. Jack.

Jack. I can't overemphasize making sure you are doing the kegels RIGHT. (I wasn't!). The two biofeedback session (covered by Medicare!) are what I needed to do them right. It was 5 months or so before I no longer had leaks. I can't remember if you had biofeedback or not. And I'm sure I mentioned that Dr. Das agreed with me that two biofeedback sessions should PART of any after HoLep treatment plan. Trust me, it worked! (for me anyway(

Had the Holep procedure around 10 AM on Thursday, March 19th at the hands of Dr. Amy Krambeck and team at the Mayo Clinic in Rochester, MN.  Rough few days following the surgery with catheter blocked by blood clots and return to the hospital for irrigation, but by the fifth day (Monday, March 23rd), I was discharged from the hospital without a catheter.  After a couple of discouraging days of almost steady leakage, the problem seemed to self-correct, so I headed for home (upstate NY).  I started the kegel exercises to strengthen the pelvic floor.  It is now Monday, March 30th.  I have the following to report: normal (very slightly painful) urination with robust flow, normal starting and stopping, no blood in the urine, no drippage, although a very occasional squirt if I cough.  All in all, not a pleasant first week, but excellent results in every measurable respect.

Great report. Can I suggest that you either sign in when you post or use a first name to sign your posts so we can be a little more "personal" when replying to your posts? And keep track of all those here having recent operations. Bet you are loving that "robust flow". Get some pads and move on with your life. Keep up the Kegels. And give it about 3-4 months from surgery and yiu should be fine.

Actually I haven't tried any Kegel or Bio-Feedback exercises. For the Bio-Feedback, should I contact the urologist to see if they offer these sessions? Thanks.

Yes. Jack. A nurse will do it and it is covered by Medicare and Supplemental. It takes maybe 30 minutes. The first time she teaches you how to do and you see it on the screen. Then when you think you understand you go home for a week and try it and come back for one more session. That's the only thing that worked for me. I was not holding the muscles like I should have. Seeing on the biofeedback monitor I knew. Again, some folks may not need the training; I did.

Now 9 weeks post op. Burning after urination is reduced and almost gone. Slow but steady improvement in stress incontinence - I keep pretty dry when just walking most leakage when bending, lifting, and twisting etc. No real issues with urgency, and I can hold it pretty good when a restroom is not convenient. Kegels 3x day.

Hi Don: I realized that you have surgery with Dr. Das on Monday. (I thought he only did on Thursdays and Fridays - but that was two years ago. Just wanted to wish yiu the best and hope you'll report in when you can. Seriously, this surgery will change your life - hopefully for better.

My father is with a very large prostate. He also hopes to have Holep. I would be greatful hearing from you about your father's experience. My father is about 75, also physically fit.
