It is right. Just this week I had to visit the doctor and without a concern I went and had to apply an injection, I realized I was without underwear. I felt normal after, but before I was terribly concerned about what he would say, but he did not even mention it.

I'm 30 years old and have been freeballing since I was in HS. I've gone back and forth with underwear over the years but it's always so uncomfortable.
