Hello everyone,

I have read almost every article about the subject from your forum and noted the facts I think which are determinants. Below are all the details of my case.

- I have a 2 years old cyst,
- The cyst is attached to my right testicle but can slide little bit over the surface of it, like a kind of bag over the testicle,
- The cyst is situated into my scrotum exactly up on the head/lobules of epididymis of my right testicle (noted during an ultrason dopler test, and also can be felt by hand),
- The cyst aches only when you press on it,
- Its diameter is 2cm (noted during an ultrason dopler test),
- All other results of the ultrason test are physiologicals (normal),
- For over the cyst came into existance, there is no apparent size change of my testicle (excluding the cyst),
- After the cyst had its actual size, it pushed down my right testicle exactly of its size, which created a difference of 2cm between standing levels of my testicles (heights of my testicles from a determined point below),
- My masturbation habit is once a day, and differs rarely,
- Just before and after the ejaculation, the cyst (and I think the epididymis) gets harder and very tightened,
- Before I had the cyst, the ejaculation was starting in once and, ending after 3-4 normal and easy spurts. After the cyst had its actual size, the pressure of my ejaculation decreased. Actually, 1-2 seconds before the ejaculation is starting, my semen is starting to flow with no pressure, after that I am having 1-2 spurts with pressure (but less pressure than before the cyst) and the ejaculation is ending again with a non pressured flow of the semen,
- Before/during the ejaculation, my right testicle is pulled/tightened down (on the contrary of the left one going up normally) and straight inside of the abdomen (but not getting inside completely),
- After the cyst, my semen quantity has not decreased, and it is 5mg,

On the ground of all the above facts, I believe that I have a hydrocele or more exactly a spermatocele. If we consider that this is a spermatocele, in an article I have read, the only cause of it was determined as a blockage of one of the sperm canals between the testicle and the epididymis.
I am worrying that the cyst damages my ejaculation mechanism because of the reasons I stated above about the changes in my ejaculation habits.
- If this is a sac the sperm is filling inside because a canal is blocked, what is its limit? I mean whether there is a risk that the sac exploses due to the pulling/tightening moments above?
- If we decide to move it away due to the fact that it is disturbing me and it can damage my ejaculation mechanism and epididymis, is there a solution for the blocked canal?
- If there is not, where the sperm actually filling in the sac will flow/fill after the operation?
- What is the risk level of the operation on my fertility and the testicle?
- Even on your opion, epididymis and ejaculation are not related, I think that there is a relation between change of my ejaculation habits above and this cyst. What is this relation on your opinion? If you insist there is no relation between them, what is the reason of the change of my ejaculation habits and the pulling/tightening moments above?

Thanks and Regards

Hi there Guest, are you concerned because you have a cyst on your testicle or do you have al ump you have not identified? It would be a good idea to see your doctor just to make sure it's okay. Can you go and let us know how you're doing?

I have a cyst on my testicle and it hurts when pressure is applied to it (wearing pants for example) or sitting all day or driving long distances.

I have had an ultrasoud done on it and they said it was a sebaceous cyst and just ignore it. But it causes pain and I wondered if I should have it drained/removed?

My husband has had a spermatecel for 15 years. His was diagnosed at 18 years of age, and it can be quite painful. Surgery is not a great option due to the fact  that it is hard to get down to the original layer, or source, of the cyst and stop it. Dr's have said that sometimes it will still occure (a leak) and thus cause two such lumps to show up on each side of the repair site. That seemed crazy to him and we had many questions/concerns, but opted out of a surgery that didn't even promise a solution. We have easily created a family, 5 kids in fact, without any affect to his fertility. He has recently discovered another harder lump wich we will be having checked out soon. Hopefully just another harmless situation.

Yeah, you've got a paradidymal cyst.  It's the persistence of an embryological remnant of the mesonephros that can become cystic, and if filled with sperm, is termed a spermatocoele.  If it bothers you, a urologist can surgically remove it.  If it doesn't bother you, don't mess with it.
