I currently have fibromyalgia (fm) and before I was diagnosed, I only take ibuprofen. The meds only helped a little but were good enough back then. Finally, I was diagnosed with fm and was prescribed ultram. I was blown away with the relief it gives me from the pain. I mean it’s not totally removed but way much better in managing the pain than ibuprofen. My question is that it only last for around 4 hours, though. Do I need a dosage adjustment? Thanks.

I'm glad that you're finding relief, however I am a little concerned that your doctor has leapt from ibuprofen straight to Tramadol and seems to have missed out steps in between.

The 4 hr relief that you are describing is the norm for Tramadol, and I would certainly advise that you and your doctor think carefully before committing to higher doses. It is quite easy to become tolerant to Opoids, and they do have significant side effects all of which have to be weighed up and considered along side the benefits.

Ther are many different ways to mange long term pain and referral to a chronic pain clinic may be worth considering to enable you to manage your health over the longer term.
