First and foremost--have you seen a doctor about this? I know that chronic nosebleeds are a difficult thing to deal with, so a doctor can offer a lot of insight. Often the bleeding is caused by dryness, so you can try a nasal spray to keep it moist. Of course, the nasal spray can contain saline and it can actually make things worse. Another thing you can consider is moisturizing your nose by gently inhaling a bit of steam. Either way though, see a doctor about this! Best of luck to you.
