62 year oold man takes warfarin 6mg daily for dvt he also takes captopril for hypertension and occasional ibuprofen for pain his dentist prescribed miconazole oral gel for a suspected fungal infection 6 day ago
what drug interaction could account for this presentation?
and what should be done

Warfarin and Miconazole have a known drug interaction.

Notify the dentist and his doctor. Unfortunately this type of situation occurs often. His pharmacist, if prescribed and filled at the same location, should notice the interaction.

In a nutshell, doctors (and dentists) prescribe drugs but they often don't understand what they are prescribing and how they interact.

Pharmacists are more knowledgeable as to how drugs work and interact with each other. In my opinion, they should actually do the prescribing based on a recommendation by the doctor.
