I really feel for you because I am in the exact same situation.
I gave up before for about two years. I hadn't touched it at all. I thought I could take it 'just the once' when I was out at the start of July and it is now two months later and I have been taking it every single day, I even eat and sleep on it. Planning on properly stopping this weekend again.
To be honest, there is not much that you could get from the doctor. You're best bet is just to book a couple of weeks holiday off work and sleep, eat and drink water. Try and eat as healthily as possible. The first couple of weeks are hell but usually into the 3rd week ,you will start to feel back to normal again. If I were you. I wouldn't take the anti- depressents at all as your mood levels will be all over the place as your body adjusts without the speed. Just accept that you will get emotional and depressed and cry at stupid things and get angry and shout at nothing for a week or so...try to remind yourself that it is just withdrawal..
If you really want something from the doctor, I would ask for a medical check over. Depending on how strung out you are, perhaps a muscle relaxant like diazepam (2mg) they are weak and will let you get a good sleep and relax.
I would also be careful what type of anti-depressents you have. Some will react very badly with the speed. I just wouldn't take them at all. Once the speed is out of your system, you will find that you become the person you used to be. You will be on time for things and will have motivation to do things you haven't done in years.
