I have had problems with perineal tearing for 30 years i am almost 50.
When i have sex or even sometimes during deficating if there is any stretching. I have used hydrocortisones, femarin, vaginal creams,vitamin E , nothing works. The area is always itchy and i catch myself scratching during the night. Sometime the area burns as it it so inflamed. I had a surgeon that insisted on a "Fentons procedure" essentially widening the oraface some 14 years back as he thought it would help with penetration as if it were due to "technique" issues. This did not work. This is a severe condition and prevents me having sex with my husband often. I have had this investigated many times and nothing ever fixes it. It was once diagnosed as "lichen sclerosis" (forgive my spelling) and given the appropriate cream but this did not help either. I have no problems with lubrication or arousal. I am not in menopause. I am desperate to solve this before it drive me crazy. please help.

You and I have the same exact symptoms. My perineum has itched and gotten imflamed/bled/oozed in the same spot off/on now for over 10 years.
I'm now 48 - not menopausal - have always had a regular cycle and keep the area clean/dry (I use a hair dryer on cool to dry it immediately after bathing) and use a fragrance free bath soap. I wear loose undies as thongs "kill" me and and after seeing MANY doctors do not have any answers. I've tried hormonal cremes, topical steroids, grapefruit seed extract, anti-fungals...you name it, I"ve tried it! But it always comes back and makes like absolutely NO fun. I"ve forgotten what it's like to have sex without dread of the pain. And yet there are times when it clears up completely for a few weeks and at 2 years ago it was gone for almost a year - I was amazed. Yet I hadn't changed anything in my regime at all.
