My son has been off for going on (just started) his 6th day cold turkey and he is very paranoid and believes that everyone is out to get him. It is very sad and hard but right now he still seems determined even though he is paranoid like crazy.<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />

I have detoxed cold turkey 8x...it can take a year before your boyfriend feels like himself again; maybe 2. I have been on many clinics which only treat you like you are a monster, and you must work for your take-outs or you will go everyday except maybe on a major holiday like today (Thanksgiving) After detoxing 8x...I now get 10mg tabs from my private Dr. who never makes me feel guilty); and now I thank God for methadone, especially after I did some research and found that the 2 scientists that invented methadone likened it to insulin to a diabetic. There is no euphoria, just no feeling like you want to die; therefore, if you can get it from your private Dr, do so, if not.....then a clinic might be the only answer. Also, when I was in Australia, a guy that lived next door died from not taking his insulin.....so, if you need it....take it.

I agree, if he can get alprazolam (Xanax) or clonazepam (Klonopin) then that would be a great help. I was prescribed zopiclone (the S- isomer is sold as Lunesta in the USA) and found that helpful. After acute withdrawal, the hardest things to deal with are (1) the insomnia, for which benzodiazepines or "Z- drugs" will help and (2) the anhedonia (lack of ability to feel pleasure) which can be helped by antidepressants.
After quitting methadone, almost cold turkey (from 110 mg/day to 0 in 8 days), it took me about 5 - 6 months before I really felt "normal" again.

First of all, since methadone has such a long half-life, you may not even be noticing methadone withdrawal. Secondly, even if you're on over 100 mg/day, a 25 mg dose will often be enough to prevent you from getting physically ill, even though you won't feel satiated.
When I came of methadone I'd been on 110 mg/day for about 2 years. When I stopped, the days were like this: 110 mg, 70 mg, 40 mg, 30 mg, 30 mg, 25 mg, 24 mg, 24 mg, 0 mg. I didn't even start to feel ill until about 60 hours after my last (24 mg) dose. That gives you an idea of how long its half-life is.
The worst day was the 4th day (i.e. 4 days after my last dose). 70 mg diazepam on that day barely was enough to get me to lie still. Withdrawal (i.e. the "acute" or physical stage) lasted until the 10th day. It did, however, take about 30 days before I was able to sleep more than 4 hours a night without prescription sleeping pills (zopiclone). I was about 50 days before I was able to have fun again and about 150 days or so before I really started to feel like myself.
Two of the doctors at the methadone clinic told me that I was basically crazy and had a 99% chance of failing because I was dropping my dose too quickly. The doctor I usually saw originally encouraged my to drop from 110 mg/day to nothing immediately (i.e. "cold turkey"). I'm glad I decided to do it in less time, rather than waiting 6 months to 1.5 years like the two other doctors suggested. Those 7 days of withdrawal sure were not fun, but in retrospect, it wasn't that horrible.
All those who claim that methadone is the "hardest" to withdraw from don't have a clue as to what they're talking about (and, indeed, many of those I've heard say that have, in fact, never even tried to withdraw from methadone).
It's basically a pharmacological fact: since methadone has a much longer half-life than many other opioids like codeine, oxycodone and diacetylmorphine (heroin), withdrawal lasts about twice as long with methadone but withdrawal is also about half as intense.
I think part of the reason people say methadone is harder to get off is because they take a higher dose of methadone than they need. For example, I was addicted to codeine for 4.5 years before getting on methadone. I was taking 1600 mg/day of codeine phosphate (roughly equivalent to 160 mg of morphine or 80 mg of heroin - and if you're a heroin addict and you think 80 mg/day of heroin isn't much, I'll remind you that it is REALLY unlikely that your heroin is anywhere near 100% pure - heck, it's probably not even 50% pure). I was satisfied at about 50 - 60 mg of methadone but went up to 110 mg/day because (1) I enjoyed it (those who say it causes "no euphoria" are mistaken for a few reasons) and (2) the cose was per dose not per mg.
The reason I say that those who claim methadone doesn't cause euphoria are mistaken is very simple: they were probably addicted to opioids before taking methadone. If you take opioids orally, then you'll find that other opioids taken orally simply won't produce euphoria once you've built a tolerance. That is to say that the euphoria is one of the first things you build a tolerance to and one of the last things to go. If you're already tolerant to opioids and then start taking methadone then of course you're not going to experience euphoria from methadone. However, if you give methadone to an opioid-naive person or if you seriously overdose on your own methadone, then you will experience euphoria. It won't be anywhere as intense as IV injecting opioids, because the rate at which it climbs in your blood is WAY slower than IV injection, but it still produces pleasure.
