An ultrasound is a very good tool for diagnosing many abnormalities in and around testicles and scrotum. It can show testicles, epididymis, even prostate. It should have shown all the swellings, abnormalities, torsion or strains, lumps or tumors.

Pains you described look like epididymis infections that are caused by bacteria and easily treated with antibiotics. There could be a lump or a swelling, maybe even a fever and other symptoms but maybe it was too early to see when you had ultrasound ( I don’t know).
I can’t tell you more although I did have problems with epididymis before and had it successfully treated. I reckon that an urologist will take some samples of your urine, sperm or prostate fluid to check for the presence of bacteria.

If you get more info from an urologist, feel free to share them.
