Some girls/women have already lost their "virginity" by other means. The hymen is a thin membrane/piece of skin that is torn during your first time having intercourse. Sometimes it can be torn when using tampons or through exercise an sports, and if that has happened then obviously it is not there to be torn or bleed when having sex for the first time. I'm not entirely sure, but I have also heard not all girls are born with one.
