In 1986 I had a Hernia operation and now there is a hard ridge at the bottom of the testical. I woke up one morning and had a bad pain on the same side I had the operation on back in 1986. the testical hangs down futher than the other one. I went to the Doctor and he said that it was not a Hernia , and there was no mass on the tastical. Any comments would be great.

Look, it is very hard to say without examining. We can only make some guesses but that won’t do you any good and we can’t say it is not that or that when we have no access to your genital area.
Has the pain occurred just once and then disappeared? Or has it been going on for a few days?
If it were just sudden and went away that day, it could have been something transient whose cause couldn’t even be found. If the pain persisted for a few days, it could be some sort of infection. It could be something else like kidney stones if you had additional symptoms of back pain along with testicular pain, etc
Like you see, these are just guesses and without a proper treatment, nothing much could be said. It would be best if you saw an urologist. He could run some tests, or order an ultrasound if the pain persisted.
