Are they normal and do they disappear by themselves? Also, do they affect fertility?

Spermatocele are cysts that form on epididymis, a long lumpy tube that lies on both of testicles. Its purpose it to store sperm before it is ejaculated.
Cysts that form on epididymis are harmless and consist of some milky fluid and dead sperm cells.

They usually don’t cause any trouble, pain or any symptoms except this little bump that is usually situated on top of epididymis. They are left alone, as they are, and are being removed only if they become troublesome or too big or if the patient specifically wants to. They can get big and annoying and make clothing uncomfortable but they rarely do so.

They will not go away on their own, without a treatment. If a person wants to remove the cyst, he needs to undergo a procedure called spermatocelectomy.

Spermatocele don’t cause fertility. Very rarely can they actually enable enough blood to reach penis but it is very uncommon. Spermatocele, on the other hand, are very common.
