I'm a 32 year old Hispanic man that has been experiencing the same issue off and on for almost 6 years now. First episode was about 6 years ago after some sex, not particularly rough but the position is where I noticed the sharp pain when climaxing, then straight blood and blood clots soon after. Eventually it heals itself but not usually until after a few days to a week of painful, bloody urination followed by clots (and sometimes what seems to be a extremely small piece of skin).

I've been to the doctor and a Urologist about it and i've done urine samples and even had a procedure that was recommended by the Urologist to open up a supposed blockage in the urethral tube. This was based on the fact that a had a below average urine flow according to him. I'm still not sure what the procedure had to do with the blood/blood clot after sex but whatever. Today 3-12-16, i just experienced it again after more than a year, this was after sex hours before. What i did notice as a determining factor was that when this first happened to me 6+ years ago, i was having sex with my wife in doggiestyle position and had no problem while i was inside her but i noticed some pressure underneath my penis while inside. I felt like her public bone (or some bone/muscle) was pressing against my urethra while i was climaxing. This hurt like a mofa!!!
