The penetration on Lidocaine into intact skin after application of Lidoderm is sufficient to produce analgesic effect. Anyway it is less than the amount necessary to produce a complete sensory block. The amount of Lidocaine systemically absorbed from Lidoderm is directly related to both the duration of application and the surface area over which that is applied. For now, we can’t tell you for sure if Lidocaine is metabolized in the skin. It is metabolized rapidly by the liver to a number of metabolites. Blood concentration of the minor metabolite is negligible following application of Lidoderm (Lidocaine patch 5%). Lidocaine and metabolites are excreted by kidneys. There is also some Lidocaine, less then 10%, that is excreted as unchanged. Patients with severe hepatic disease are at greater risk of developing toxic blood concentrations of Lidocaine. Reason is that their inability to metabolize Lidocaine normally.
