Hi, I'm 21 years old, I'm sexually active and I have a problem about my testicles. Lately I have noticed a dull pain in them while ejaculating and sometimes even when urinating. When I sensed that there was something wrong I tried to examine my testicles with my hand and discovered that they hurt in the back end. When I ejaculate there is a little blood in the sperm. I'm very scared about this whole thing, does anybody know what is this disease that I have and how can it be treated?

There can be many causes of testicular pain and discomfort. If you are sexually active then it is possible that you have got some kind of a sexually transmitted disease, especially if you have had unprotected sex in the near past. If you are experiencing pain during ejaculation and urination, your testes, urethra, epididymis, or even prostate can be inflamed. The cause of the inflammation are most likely some kind of a bacteria that you have contacted either by practicing unsafe sex or by bad hygiene. I think that epididymitis is the most probable cause of your pain, since you say that the pain originates in the back end of your testicle, where your epididymis lies. Epididymitis as any other bacterial infection is easily treated with antibiotic therapy. There is also a possibility that you may have testicular cancer, in that case you should try to feel a bump on the surface of your testicles. In any case you should immediately go and have an appointment with your doctor and let him examine you and give you proper care and advice about this problem.

I know this thread is ancient, but it is still in the top 2 results on google, so I am going to post on the topic anyway.
I had a similar problem when I was 18, that my "Urologist" told me was a "torsion of the testicles". Basically, this means that my testicles had become intertwined and they were being starved of blood/oxygen. I had to have surgery, which is very commonplace to relieve the pain. There was no pain afterwards, and only slight discomfort from the incised area but that healed after 2 weeks. (If you think you might have this problem, seek medical treatment immediately within 6 hours of the onset of pain! It feels as if someone has your testicals in a constantly tightening vice grip and is impossible to ignore.)
I'm 30 now, and haven't had any testicular pain since, until now. I noticed a dull throbbing ache in my right testicle today. It is nothing like the pain I had when I was younger, though it is more of an annoying throbbing pain. It feels as if the pain is on the backside of my testicle. I took a warm bath to see if that may help (thinking I may have inadvertently injured it somehow) but that didn't help at all. Is there anyone else that has had a problem like this? Are there any experience urologists that may be able to help me diagnose this?

If you are going to reply to give advice, do not lie to people who are searching for advice. Most likely, you need antibiotics to treat a common infection, which could have been caused by a number of reasons.
You do not need to see a urologist, in fact because a urologist is a specialist you can only be seen by obtaining a referral from a family physician.
STEPS: go to see your family physician, they can prescribe antibiotics that you need, furthermore if more treatment is necessary, your family physician will refer you to a urologist.
