My testicles thighs and nipples are in bad shape

Hi there, i am 18 and about a year ago i suddenly had a really tempting itch on my testicles. as i went on the days i scratched and rubbed untill the top skin was off, they were bleading for a bit and then pussed out , but later just oozed out some clear fluid. a while later, the same happened to my nipples and the dark area around it. and when they dried up, they were easily broken off by the dried split top skin.They took a long while to dry up, but now, they are not healing back to their initial smoothness. I am a virgin and i am at the stage of getting intimate. they occasionally itch and hurt from the minor scarring. I've been asked to use ELECOM and my prescription is done. what should i do, what is it? cuz i am a bit scared to actually show it to a doctor.
