If I go more than a few days without ejaculating (either from masturbation or sex), I will begin to spontaneously experience rather severe groin pain. Initially it feels like a normal urge to urinate, so I will go to the bathroom and void. However immediately upon finishing and zipping up, it will feel like I have a very urgent need to urinate much worse than before. Trying to suppress the urge is extremely difficult and painful. Of course nothing will come out if I try again, but all discomfort will cease as long as I am relaxing the muscles as if I was trying to urinate. This pain will usually subside on its own within 30-60 minutes, but it is quite uncomfortable to the point that I have trouble concentrating or talking with people while it is occurring. If it happens at night I will not be able to sleep.
Since it's been happening with such frequency lately, I've begun to notice even the faintest hint of it creeping up (a slight pressure in my testicles/prostate area). If I masturbate right away it will generally resolve without any problem. If I wait for it to flare up to the point that it feels like I desperately need to urinate as described above it will still be extremely uncomfortable and painful but will generally resolve sooner than if I didn't masturbate.
I seem to remember this happening every once in a while when I was younger. However I do also recall being able to go weeks without masturbating. Now, however, it is guaranteed to happen if I don't ejaculate every few days (I am 34).
Has anyone else experienced this? Is this worthy of a trip to the urologist? I remember asking my general physician about it years ago but he dismissed it as related to dehydration, which I am certain is not an issue for me these days.

It MAY be epididymitis. You have some of the symptoms. It's a bacterial infection and can cause groin pain, discomfort. You can also experience pain/discomfort when urinating.
It's very common.
See your doctor. They'll likely take a culture. Treatment is antibiotics.
