Perfect! Thank you so very much. I began Metformin about a week ago and now these "grapelike" pods are showing up in my stools. I was seriously concerned.

I don't have any type of diabetes, but did eat pizza a few days ago with my son,HEB became nauseous, next day I'm nauseous and have diarrhea, and little fatty looking balls, so that's my opinion too, keep out a watchful eye, and if it continues, talk to your doctor.
This probably sounds a little gross, but if it occurs again, take a picture and show it your doctor, or another as you have a right to a second opinion. Hope that helps.Cat LVN

I stopped metformin years ago and I still have at least one every other day. Sometimes they happen with longer pills too, with remnants of the pill still partially formed inside.

Are you on Metformin Extended Release? Then it is just the polymer matrix you see in your stool. All of the drug has been absorbed. This is a common release mechanism for certain drugs to make them extended release. If they were not working, your blood sugar results would be sky high. Here it is in the literature:

System Components and Performance - Metformin HCl extended release tablets, USP comprises a dual hydrophilic polymer matrix system. Metformin HCl is combined with a drug release controlling polymer to form an "inner" phase, which is then incorporated as discrete particles into an "external" phase of a second polymer. After administration, fluid from the gastrointestinal (GI) tract enters the tablet, causing the polymers to hydrate and swell. Drug is released slowly from the dosage form by a process of diffusion through the gel matrix that is essentially independent of pH. The hydrated polymer system is not rigid and is expected to be broken up by normal peristalsis in the GI tract. The biologically inert components of the tablet may occasionally remain intact during GI transit and will be eliminated in the feces as a soft, hydrated mass.

So what if they are happening 4-5xs weekly without metformin or any ER tabs? It’s all powder medication and a partial pill in the center.

I take Metformin 750 ER tablets and this has been happening to me too! THey look like large poofed up white grapes and are white filled and often float. I've taken this stuff for months and never noticed it, but lately I've had much looser stool than ever before and I think that's why I'm seeing them now. Thank you so much for your post! It really made me feel better :)
