With all your complicated symptoms and the history of your health it would be best to see your doctor. He is the one who knows you the best so he can let you know what is going on without unnecessary tests, which are usually pretty expensive.
And just on base of what you have wrote I am not sure that I can tell you the diagnosis, also I am not sure that anyone else can because it is just guessing, and it is very unlikely that someone had absolutely the same problems as you did. Moreover, your doctor is familiar with your situation, so I am sure that you are more comfortable with him as well. Good luck.
