My grandfather is 64 years old and is dealing with diabetes type 2 for a decade. He also has heart problems and I must say that he is not listening to doctor's orders. Recently, he cut his toe (while cutting the nails) and infection happened. So, is there some connection between diabetes and developed infection? What are possible outcomes?

AS a result of high sugar level in blood, micro angiopathia might happen (eyes, kidneys etc). Also, regenerating process is slowed down because of this (toe injury for example). As for outcomes, the best would be regulating the sugar level. The worse would be toe amputation. Let's hope it won't come to that.
