I've been doing it for years and I'm 50. Here's my experience. I'm a doctor, so I feel I can speak with confidence as doctor as well as patient.
I was born with urinary incontinence, so I've grown up having to wear a Cunningham clamp my whole life. Look it up, if you want. A Bard Cunningham clamp is a maleable metal clam that is covered in soft rubber and on the inside of the clamp that comes in contact with the penis, it has soft foam rubber like material. It has several settings to the clamp that allows the user to set how much pressure is on the penis. You can bend the soft metal of the clamp itself to adjust pressure on the penis further, if needed. They are durable and washable.
Having grown up using them, I'm so used to it, I don't even notice it's there most of the time. It's awesome because it gives those of us with urinary incontinence means for control.
I've tried other clamps/incontinence methods but nothing gives one control like the Cunningham clamp. They're about $32-$35 a piece.
Now to retrograde ejaculation.
When I became sexually mature, and started masturbating, I didn't know any better and to prevent a mess, just before orgasm, I'd clamp down hard on the clamp setting to prevent ejaculation. This would force retrograde ejaculation into the bladder. Then, I could get up at my leisure and urinate off the semen. My biggest fear was having a woman reject me if she found out about my problem. To be fair to my girlfriends, I would tell them about it and I never once had any of them walk away. In fact, there's a natural instinct in good women to accept you just the way you are so long as you're a good person, love her despite her flaws, and if you're into one another! The fear melted away. The women I've told were curious, but always understanding. I've been amazed at how awesome the ladies in my life were about it.
I've noticed several things about using the clamp..
1. It hasn't caused impotence in the least. Erections are fine. I only have 50+ years experience, so I can't say for sure beyond that. I have 4 healthy children, all of whom I fathered. Besides that, as a physician, I have access to a microscope and out of curiosity, I've checked my sperm several times over the years. Everything's always normal. That being said, I have a lot of sex, both with my wife and maturbatory. I have sex in one of those ways on average once or twice a day. Because I have it so frequently, I don't make a lot of sperm. I've gone on 2 week "sebbaticals" and ejaculate increases substantially. I've noticed over the years smaller and smaller amounts of ejaculate, so there's the possibility that retrograde ejaculation could be causing me that, but medically speaking, that doesn't make sense. It's also a medical fact that on average ejaculate decreases with age. A 50 year old like me will have about 20% the volume I had at age 30 and so I think retrograde ejaculation gets a bad rap in this area. I think urologists hear men complain about decreased volume of ejaculate and then look for excuses to blame it on: "Oh, you must have had mumps as a kid," or "You have had prostatits or epididymitis several times over the years so that must be it." We docs want to give an answer that calms the patient. The truth is that three things effect volume: aging, length of time between ejaculations, and hydration status.
2. It hasn't caused significant "plumbing damage" as far as I can tell. Everything works just like it always has and is normal minus my having urinary continence since birth. Because of my incontinence, with abnormal bladder shape and in the past, frequent UTIs, I have had several dye studies of my urinary collecting system over the years. There was dilation of everything from urethra in the penis up to my kidneys, and even into the ductus defferens (vas defferens) but it is undertermined if that was caused simply by me holding off on the bathroom too long (a bad habit I've had, since I had ultimate control over when I had to urinate, thanks to the clamp, and was also born with a smaller than normal bladder. So, I'd put off emptying the bladder till it was convenient. That isn't a good habit because holding urine too long can back up to kidney and cause hydronephrosis and the increased pressure over the years likely cased the dilation. My urologist used to get into me all the time about that, but I was lucky and have never had kidney damage of that type.
3. Now for the interesting part! Using the Cunningham clamp has NEVER caused me pain during orgasm/ejaculation. Sure, if a pubic hair gets caught on it, or the clamping mechanism is just so against my leg, it can pinch. A simple "readjustment" in location by moving the leg or discreet moving thing through the pants takes care of those rare events.
On the contrary, when it comes to sexual pleasure, I've discovered that orgasm, at least for me, is so much more pleasurably intense against the closed clamp than when I have a normal orgasm with the clamp off and let ejaculate leave naturally! I'm talking 2-4X more intense! I've lived many years now feeling sorry for other men. The only time it's remotely uncomfortable is if I orgasm with an overfull bladder. On such occasions, there will be a very brief dull pain in one testicle. It goes away immediately upon emptying the bladder.
I love my wife intensely! She is hotter than one would ever guess by our conservative outward appearance/actions. She's playful and up for anything. Sex with my wife is incredible, but if I had to pick which ORGASM I'd selfishly rather have for my pleasure alone, I'd pick the retrograde orgasm ejaculation against my Cunningham clamp 10 to 1.
I am not exaggerating. I'm only writing this to educate men, as I've looked before on line about this and could never find info about the positives of blocking ejaculate. As a matter of fact, for me at least, there's great temptation to simply masturbate all the time and that would be unloving and selfish toward my wife and unhealthy for our marriage.
During fallatio, I have to choose to proceed with or without the clamp on. Both ways are great, but the clamp not only intensifies my pleasure, but makes things lots less messy for her. I wash off the clamp and penis before hand to give it no odor and usually leave the clamp on, just moved to the very base of the penis to keep it out of the way. If the pants are on the clamp is hidden anyway. Most of the sensation that gets us guys off is about the frenulum (underside of the penile head), so the clamp isn't a problem.
We all fantasize about deep throat fallatio, but the business end near the head is actually where it all happens. In addition, I hate the thought of her choking/gagging. Why ruin her experience? Sperm, no matter how much the porn industry or even she lies to you about it, isn't exactly tasty/delicious. It can be a turn on for you both, but there's no market for it outside of highly charged sex or people would be selling bulls sperm on the market - gross! The clamp keeps her from having to get sperm in her mouth.
Personally, I find "facials" distasteful and degrading, but to each their own. If I wanted to give facials, the clamp would actually be a positive in that respect. I could ejaculate then release the clamp and give a perfect one. In porn, that's what the actors often do anyway, just with their hands.
I've even left my clamp on during sex to heighten my ecstasy, but as it takes up about a half inch of the penile shaft and feels a little strange up against the pubic bone during thrusting, so I usually take it off.
I consider my sex life healthy. My wife has never complained and we are very sensual/sexual. No matter how you have sex, if it's with the woman you love, it's incredible! I consider myself the luckiest guy on the planet. The clamp just has never been an issue and in fact, I'm greatful for it!.
