Sounds like retrograde ejaculation.  You ejaculate into your bladder.

Are you on any medications at all?  Do any drugs?

I believe that your body is still sorting out when and when NOT to send your sperm through your penis. It's a kind of complicated system in there, specifically designed to keep you from mixing urine and semen, to ensure reproduction. But sometimes guys CAN send urine through their erection, and CAN mix urine with their cum. If they push hard enough, they can overcome the "switch" in the prostate gland that controls these things. Rather than a "retrograde" ejaculation, it is possible that you are not ejaculating fully or powerfully enough to expel all your semen when you cum. Then when you pee afterwards, whatever is still sitting in the system will be washed out with the urine. Some guys can also find that they can cum more than once in a short period of time and at your young age, your body and mind (because orgasms REALLY occur in the BRAIN) are still sorting out the timing. I wouldn't worry too much about it. Give it time. Masturbate as often as you comfortably can, paying special attention to the mounting sensations as you get close to orgasm, AND to the sensations and physical results as you "come down".
