I'm on the list for a 2nd kidney transplant, a gland under my jaw is swollen, do I need to go to ER?

Hi I'm a 37 year old woman with stage 4 almost stage 5 E.S.R,D (end stage renal disease), I'm on the list for a second kidney transplant, l had my first kidney transplant almost 12 years ago, this time around my doctor is trying to keep me off dialysis, and I'm hoping the same thing. Last night I noticed I was hurting right below the right side of my jaw and that gland is swollen, the area over my liver, into my stomach and back, up my chest, and into my hero graft(which I smashed in a door in the cabinet in my bathroom 3 weeks ago), has been hurting, and  my body is having uncontrollable jerks, I can't hold on to things very well and my mind is foggier than before, so I guess what I want to know is should I go to the ER or not?!?
