The frenulum beneath my penis looks thin as if its been worn away, I am uncircumcised and sexually active. It hurts to pull back my foreskin. Should I be worried?

I don't think there is anything to worry about but it may rip while you are using it. That might not be pleasant but it would solve the problem. Mine tore several times when I began having sex. The first time the ends healed together and it was shorter than before. After that I put some TP on it to keep the ends from going back together when it tore. After a few times of that it was as if there had never been a frenulum there and sex has been a joy ever since. Because my foreskin now goes all the way back with no discomfort and it slides back and forth easily while inside her my wife is very happy with that. She says it feels great.

As aushi said you can go have it cut to solve the problem right away. I would only caution you to be sure he/she knows you want it GONE so your foreskin can slide all the way back. I have read too many stories here about guys having this done and it still being too short for comfort.

I promise you will not regret getting it fixed so your foreskin can slide all the way back easily. It allows your inner foreskin to get stretched during sex, without any pain and that stretching of the inner foreskin is where most of your pleasure comes from.
