Yes, depending upon his age, several things could be happening.

When a boy's testicles develop, they decend into the scrotum. If the opening where they came from doesn't completely close, the testicle could pull back up into that channel. If it stays up there, the sperm is likely to not be fertile.

Therefore, he sould be examined by a doctor to determine if the testicle has retracted up into the body, or if it is still in the scrotum.

His age will play a large role in determining what to do. If he's 34, that's different from age 4 or 14.
