pain in testicles, it hurts like a hell and a bit of liquid came out

hi , I'm almost 20yr , i use to have drugs since last 2 years but i have reduced the quantity of them, i use to took pills , cough syrups , painkillers , and marijuana . i haven't drunk ever nor used cocaine heroin . since last 8 month my testicle have started getting tremendous amount of pain in it went upto the abdominal area , and then i notice i got enlarge testicle (right testicle only) and also many times even when i urinate a bit of sperm came out ! and it pains a lot ! so i reduced the quantity of drugs a lot. but from last 2 weeks my left testicle has also started the same thing , it doesn't hurt as much as the right one does when the pain start but it does , hope you understand what i mean . i am really very scared , even i told my parents they also doesn't give that much notice to my problem . i cannot bear this pain anymore . and i would also like to tell when i paint's off with my girlfriend suddenly after 2mins my testicles start pain like hell and a but of liquid came out in my undergarment , i doesn't told about this to my girlfriend at that moment . and our special moment was messed up because of me , i made some excuse that i really getting late i got to go now etc..... plz help me as soon as possible , I'm feared that i am going to loose my manhood .
