Enough of that...yes, the skin can tear and it can be messy. You'll probably see a bit of blood and have some pain. I'd recommend that you use a condom, even if she's on birth control, and lots of lubrication. It will help keep your skin from tearing and it will also slow you down a bit. As you get older it should loosen up some more so it's less likely to happen.
As for the spots, they are very common. From your description it could be pearly penile papules. If you haven't had sex, then you most likely do not have an STD. They could also be from hair follicles. The best thing to do would be ask your doctor next time you see them.
