itcy under scrotum in between my scrotum and rectum

I am single, bisexual and i have hemorrhoids, at first the itch started at my rectum and eventually went in between my scrotum and rectum. it is so itchy and the more i scratch it the more i feel like i dont want to stop, because it eases the itchiness until it bleeds. and then after 2 months i noticed a white pimple like under my scrotum and it starts to multiply. it is not very obvious unless i will look at it intently. but i can pop the white pimple like off. i am applying an anti itch lotion calamyl twice a day for a year now, but if i stop applying the lotion, after one day the itch will come back again. I already had an HIV test 6 months ago and it was negative and I did not had any sexual penetration until now. the sexual contact that i only had was oral sex. i am a bit nervous about this terrible itchiness and i dont know what ointment should i apply so this will be permanently gone. i hope you can help me.
