I am a 56 year old man who has always had a great sex drive and enjoyed fantastic sex. My wife and I have always had an excellent satisfying sex life but just lately in the last couple of months the sensation of orgasm - the huge bang is no longer there.

I don't have any erectile disfunction and the sex is varied and exciting. I get turned on but the feeling at orgasm just isn't there. I now find I have an uncomfortable feeling in my testicles after ejaculation which takes a day to settle down ( no more twice/ three times a day for me.) This discomfort travels down the inside of my thigh on my left side. I am also urinating more and having trouble controlling the stream ( aiming)

Is there any reason for this, who do I see and is the problem temporary? Any suggestions on how to get it back.

My wife thought it was related to Ibubrufen I took for a week when I had a muscle problem in my leg. I stopped the tablets and there was a marginal improvement but not enough. Could this have damaged me in any way? She also felt a small knot at the base of my penis like a knot, which sometimes is there other times not. Is this relevant?

She suggests a period of waiting a couple of days but having lots of foreplay to see if that will work. Any help or advice on this would be appreciated. I don't want to lose it and I do use it!

First I would suggest taking a few days or even up to a week break from any sexual activity. Then give it a go with some foreplay and sex and see if things remain the same. If you think the problem is persisting, I would see a doctor.
