Lump in right testicle and two lumps on lower back near kidneys

I´m in need of answer : My boyfriend is 38 years old , and he is complaining on pain in lower back and that two lumps have appeared that are painful , now there is a lump in his right testicle . Whole day he have spent with nausea , and he feels tired  , for past two days  he only sleeps , looking exhausted . He is pretty scared , can understand why . He had bladder infection two years a go , and that was his first time .

I will make an appointment with doctor tomorrow anyhow .

So ,can it be cysts , can it be anything else than cancer ?

Can it be that his overall health is bad and that his immune system is fighting ?

He have spent last 3-4 months working a lot and sleeping just 3-4 hours and being awake for longer then 24h .
