i am 30 years old and ive had this issue for a while. As far as i can remember, i have had several hits on my right testicle at younger age. It got to the point where im always afraid of anyone hitting me there or grabbing hard cause it will hurt. I saw a urologist last year and he recommended wearing briefs while playing sports. But now i can only wear breifs otherwise that testicle will always be itching.
At the same time, sometimes both tescticles would be relaxed and feel smooth and at some point they shrik and create a weird feeling.
The urologist gave me some antibiotic saying that there is some inflamation but i didnt take.
in short what worries me is that im always in fear and sometimes i feel that when that testicle is in pain, i get pain on the right side of my back.
any recommendations?

I can just imagine how it feels when you have this fear of hurting your testicles present all the time. Especially because you have been hurt as a kid and I bet this remained your lifelong trauma, didn’t it?
Anyways, you now have to take good care of your testicles, especially you have to be cautious when are playing some rough sports.
If you have already been to the doctor and he prescribed you the antibiotics, I wonder why didn’t you take them? This would surely relieve this weird feeling you had, and if the doc said you had the inflammation, it would have be gone so far. How do you feel now about it? Do you have some stinging sensations in your testicles? You just need to take good care of everything that is happening concerning them, you wouldn’t want to be left without them in any way, would you?
What I suggest is to go back to your doctor and talk to him once again, let him examine your testicles in order to check if there are some possible lumps and then set an adequate diagnose and decide on the following treatment, if necessary.

Let me get this straight...you're concerned about pain in your right testicle and you saw a doctor, and he prescribed an antibiotic to clear up the swelling and infection that appears to be present... but you DIDN'T TAKE IT????

I agree with DanDan... you need a thorough examination to see if there are cysts, lumps, bumps or lessions. If there is a structurall problem, an ultrasound is probably warranted.
But these are all concerns that a urologist can address. If you want some help, start with your family doctor, and he'll make a referal to the specialist if he can't help you. But we need more info to learn why you failed to take the antibiotics that he meant for you to take. Why not??!!
