Hello all, this will be a fairly long read as I want to include as must info as possible to get the most accurate advice. THANK YOU to anyone who takes the time to read this (particularly those with medical expertise) as I am literally at a loss. I am a 25 year old white male who is experiencing a variety of symptoms, particularly in the groin, testicles and penis. I will give you some background info, the symptoms, what's been done, what's in the works, and then finally my request for advice.
Years ago (when I was 17/18) I had dealt with 2 bouts of what physician's called 'NGU' or non-gonococcal urethritis. This was apparently given to me from a woman I was with, strictly from oral sex. Anyway it was subsequently treated with cycles of Azithromycin and Doxycycline which resolved the issue(s).
Since then, ANY time I feel any discomfort in that area, I default to thinking it's an infection. I am sexually active, and at times have not used protection but it's been with women I am dating/with, not some random bar hookup...and in an event I am not monogamous at that particular time, I use protection. Due to this I frequently visit clinics and get checked for the most common conditions, chlymidia, gonoorea, syphilis, etc. Every time I have went in the past 5 years I have not came up positive for any of the above.
I have visited my PCP and explained to her that I have a sedentary job and I'm not sure if it's the cause or worsens the symptoms, but here's what I am currently experiencing:
On and off 'aches' or sharp pains in the groin, particularly the left side. This can also occur in the testicles, and I at times have had pain in the tip of the penis and throughout the 'shaft'. PCP did physical checks of the testicles, nothing stood out. Due to a high deductible I have to save money first before I can get certain tests, then go for said tests. I had a CT scan done of the abdomen and pelvis, which showed nothing aside from a small intestinal malrotation, which my PCP said was probably congenital and shouldn't be causing me any issues. I have, at times, been prescribed Doxycycline cycles for a different issue, a skin condition which we believe to be folliculitis which appears on my arms, inner/outer thighs, buttocks, neck, penis, you name it. It's particularly visible on the arms and almost always is a hair follicle with puss, nothing crazy basically like a small pimple. Very hard to notice unless you are literally up close inspecting. If you pinch or pop it, usually a hair comes out and the area becomes inflamed for a few days, will scab over and eventually go away. I have had success with exfoliating, using lotions, tanning, drinking lots of water, etc. At times, it does flare up.
Anyway...I mentioned that because of the antibiotics - if there was any underlying urological infection, the doxy would probably wipe it out as well. Multiple urine screens over the course of time have shown that my urine is always clean. Usually when I have an infection, my skin becomes reddened and very sensitive, which is why recently it has led me to believe I have an infection - but alas, blood tests have been done, urine screens - nothing out of the ordinary. Now I AM borderline HBP, with high cholesterol. Could be the source of the sensitive/red skin. Very odd for me being I eat somewhat healthy and stay in shape most of the time, the BP issue is also genetic but to eliminate those as the culprits I have initiated a workout regimen to drop to a healthy weight (195/200, currently at 219) and improve both cholesterol and BP. Diet has changed as well. I try to stick to fruits, veggies, fish, etc.
Currently on a cycle of doxy for the skin condition, and within 3 days there is a vast difference in the amount of 'bumps' i had on my skin, and I'm sure after the regimen is completed 95% of the bumps will be gone. BUT...today it has cropped up again....pain in left groin. It began as a sharp pain, occurring during both sitting and standing. 2 ibuprofin took the pain away within 20 minutes. This is part of the same issue I've told my PCP about, and she has ordered an ultrasound of the groin and testicles to rule out the obvious - but with a $1000 deductible, I have no money for it right now so I'm sort of stuck. Physical examination by a 'good' urologist in my area said there was nothing out of the ordinary, didn't feel any hernias, etc. Pain like this has now been ongoing for over a year and a half.
I know I need the ultrasound, but I am already thinking it's probably not going to show anything. In the event that the ultrasound comes back negative, only a urologist can be consulted from there on out and it looks like they will be at a loss as well. I started doing some digging, and many people have said this could be a variococele, testicular torsion, infection, etc. Again, urine screens and STD tests have came up negative every time I've went. Physical examinations come up with nothing as well. Some even say it could result from neurological trauma, the only thing related was about 2-3 years prior to the pain I was a victim of a violent crime and my forehead was fractured, sliced the derma and caused pneumocephalus. It resolved itself days after, did not need surgery on the head although I received stitches and a rhinoplasty for other injuries. Being the pain began years after the fact, not sure if that can be related or not.
Lastly one thing I found too was "chronic idiopathic orchialgia", which essentially describes the pain I am experiencing to a T. Prostatitis also seems to be indicative of what I am experiencing, but I had a prostate example about a few months after the pain began, nothing noticed.
A member posted this about a year ago, and resonated with me:
"I've had a similar problem for years. The pain ebbs and flows in the left testicle. For me it's always brought on by strain and last October it became the worst it's ever been.
I've had urine tests, ultrasounds, tests for hernia, all negative. I've been to 3 urologists, a pain clinic, and 2 neurologists. I had a genitofemoral nerve block(created new soreness around the injection), a diagnosis of idiopathic orchialgia(which means "you're stuck with this"), a recommendation for a vasectomy, and another opinion that nothing can be done.
I'm taking Lyrica and Cymbalta which at least take the edge of the pain, but it's not fixing anything.
I'm seeing a physical therapist who does myofascial work (rectally) to try and loosen up tight pelvic floor muscles. She recommended a good book called "Headache in the Pelvis." So far, it's a good read and is giving me a little hope that this elusive problem could be cured.
There's also a possibility of PNE or Prudendal Nerve Entrapment, but that was discounted by the pain clinic and a neurologist in my case, but who knows, others might want to investigate this condition.
I sympathize to all those who are suffering. It affects work, relationships, mood, exercise, EVERYTHING."
I have no clue what to think or do if the ultrasound comes up with nothing. I cannot afford to go from doctor to doctor to be given 'i thinks' or come up with ideas that go nowhere, but I want to rule out as many possibilities I can before I eventually say "I have to just deal with it" and realize it's not a life-threatening condition, such as the orchialgia.
