Did you ever discover what this was? I've been experiencing exactly the same thing with my left ear. The pain just started about a month ago when I sleep on that ear but the "click" has been occurring for a long time. Almost a feeling of pressure that needs to be relieved.

A few months back, I went to a new dermatologist to have some skin concerns checked out, she asks what I'm there for that day and as I rattle off the areas of concern, she was looking at my left ear the whole time. As described by others in the post, the antihelix was bright red, the skin was flaky, it was burning hot to the touch and I was uncomfortable but it was the one thing I didn't rattle off. She admitted after addressing the other items that she was surprised that wasn't first on my list since she was certain it was painful. She rattled off the name of it so fast, I didn't catch it (but thanks to this post, I looked up CNH (Chondrodermatitus Nodularis Helicis) and that's matches it exactly). Her fix? Never ever ever put pressure on it again. As in, for the rest of my life. Learn to sleep on the other side, wear earbuds on the phone or use the other ear, no headbands, etc... That WAS the only side I could sleep on but since following her advice, within a month, the scaly skin was gone, the redness was going away and now the pain has gone completely (8 months). She did mention finding a pillow with a hollow center, but until I could get used to sleeping on my other side, I rolled a washcloth, curved it into a horseshoe shape and would roll and unroll it to create the height I needed to protect the ear without it being too much. It's not a perfect solution but after just a couple days and nights of no contact, there was definitely a difference in how that ear felt so continued to roll up that washcloth before bed. While I can occasionally sleep a few hours on that side without repercussion, I don't ever want to have that pain return so it's very rare that I will do that now
Just occurred to me that the soft baby teethers might be an option for some of you? Or the round stuffed toy rings also sold in the baby department?

I've had trigeminal neuralgia on left side for many years but ear pain only on right for years. Now I have it in left ear too and I don't get it when sleeping on either ear. It will start anytime in either ear or both. I wish I knew what would cure it.

I started doing some research and found this product -
"ears ear pillow"
probably other styles and brands out there but this is a better suggestion than a baby teether. :]

I have had this outer ear pain all my life. The pain is similar to a jammed finger. I have it only twice or three times a year, but it is debilitating. I sometimes think Vincent Van Goh had this pain when he cut off his ear. I can't help but think a shot of Novocain behind my ear would help.

I have same problem, but it is caused by clenching my jaw at night. I got a good morning snore solution device and it eliminated the problem. There are similar devices on Amazon that are cheaper if you want to check it out.

I have been having this issue for a few years now. Whichever ear I'm laying on ends up in a searing pain. I wish I could fall asleep on my back. My ENT said since I don't have an ear infection, there isn't anything wrong. I'm so frustrated with it. Glad to know I'm not alone but I hate you're going through it too.

I have just started getting this pain up on helix 1 and slightly behind the ear after being in hospital for 3 days with high blood pressure. i think there is inflammation around the helix area which is caused from inflammation around either the jugular or the carotid arteries. it is just a theory I have. One way of telling is to take a neurophen and see if the pain goes away.
