Now five days after a HOLEP performed by Dr. Krambeck. Still some dysuria, but otherwise no adverse effects, including no incontinence. Improvement in flow and frequency of urination since the surgery was performed.

Nate, from last Thursday (2/9), exact three weeks after surgery, my urine bleeding stopped, up to now, three days in a row, there were no visible blood in my urine. I have no incontinence, but a little bit leaking. Still got up 2-3 times during the nights. I am reducing the water intake especially after dinner, it should help for frequency of urinating. I have resumed my normal daily activities, and feel OK. I will get back to you after 4 weeks PO. How have you been these days?

Nate to Ben. I’m pretty much the same as you. Blood stopped about 3 days ago. I scaled back the amount of water I was drinking when the blood stopped. I now drink when I’m thirsty. I get up maybe once, some nights I sleep all night. Urine color is pale yellow. I Still have a strong stream. And I still drip too occasionally and depending if I eat something that causes bloating or get to active. Not sure how long till the pelvic muscles will take to return to normal. Seems that varies from person to person and from doctor to doctor. Dr Miller operated on me an hour longer than she expected, so it may be weeks before I’m totally back to normal on that. My return visit is on the 27th - exactly two weeks. I look forward to talking to the PA.

Does anyone know of or heard of Dr. Casey Dauw of University of Michigan? He has done over 1000 HoLeps and was trained by Dr Lingiman. I have a 150 prostate and cannot void at all. I am also 81 years old and my local Dr recommended him.

Covid test was absolutely nothing at Northwestern in Chicago. I hardly felt the swab in my nasal cavity. Results were available the very same day.

John. This is Nate. Sorry you are not able to void at all. I don’t anything about Dr. Dauw, however if he was trained by Dr Lingeman and has 1000 patients, I’m sure he’s a good choice. My Dr (Nicole Miller) was trained by Lingeman and she was excellent. I would suggest getting in to see him asap. Wish I had done mine much earlier. I first had urolift and it was a disaster disappointment. Let us know how it goes. I’m exactly 5 weeks post-op and urinating great, no problem sleeping, I dribble a bit, but improving. Hope all goes well for you.

Nate, it's been a while I haven't updated my PO status, since it's not much changed. I went Vanderbilt for follow up appointment on last Tuesday, 2/28. I went through the urine and blood test, and Dr. Miller came to see me after the PA appointment, that made feel good. My stream was strong, and bladder was empty after urinate, my PSA was dropped to 0.78 from close to 4. My urine test was all good except "UA Leukocyte Esterase E) Large". I thought I had UTI, but Tylor said "it's normal after surgery, since it's healing, the urine was negative for nitrites, which is more indicative of a UTI, especially after the surgery you had. The discomfort after urinating is expected and will last approx 3 months, but will slowly improve over time." I have little discomfort and sensation, and little more frequent urine interval, I will see after 3 or more months, hope those symptoms will be improved over the time. Over all, I am satisfied with the surgery, only hope better and the best, over the time. How was yours.

Hey Ben. Nate here. The PA had something to do last Monday and rescheduled my appointment until tomorrow. Glad you got an excellent report. Did they schedule for another appointment for sometime in the future? I will let you know how it goes.

Nate, there is an optional 6 month follow up for me, Mostly, I may communicate with them through the portal or email, unless it's necessary to go by person. How did you feel recently?

Ben. Saw Dr Miller’s PA this morning. Sounded a lot like your exam. She showed me the graph of my urine flow test and she said it was very fast. What they hoped to see. My urine also had "UA Leukocyte Esterase E) Large" and “Negative Nitrates”. I had remembered what you said about your report so no worries there. I don’t yet have the report on the PSA. I was always around 4, so I expect it to be lower. I was surprised by one report, she said Dr Miller removed 110 grams of prostate. My prostate my rectal ultrasound was exactly 147. So I have very little prostate left. I guess that’s why it’s so easy to urinate now

Nate, good to hear that you got the god result too. Your are right, with 110 grams of prostate tissue removed, you are pretty much has done for the prostate problem, as you mentioned before, Dr. Miller said to you, you may not have to worry about the urinating problems any more. Also, after surgery, you PSA should be down to below 1. I still have some discomfort, which might be the burning as people talked about. Do you have that feeling? My urine frequency is still little more than I expected, i think it's something with my bladder working. Since I had prostatitis and enlarged prostate, I was urinating frequently BEFORE SURGERY, the bladder might be got used to working more actively. I hope it get corrected after 3 to 6 month. I will do some exercise to help the bladder working less active.

Ben. My PSA was 0.75 ng/mL. I don’t have burning but I do have an odd sensation at the end when the bladder empties. I’m guessing it’s just so different to what it felt like before. It’s more like a dull pain. I still see some blood occasionally if I don’t drink enough water (just finished 7 weeks) but the PA said that was normal up to 8 weeks. I also have discomfort sitting in certain chairs and riding in the car. Do those bother you. I’m hoping that goes away as I heal. So, I carry a foam seat cut out for prostate that helps. You have that issue? As for frequency, I’ve had that but I started making myself hold it and that seems to help. When I get that feeling at home I sit down and drink water until I know my bladder is full. Someone told me years of bph makes the bladder atrophy, so we have to slowly work at expanding it again. But all in all I enjoy taking a leak and watching fly out

Nate, we have similar experiences, I feel the dull pain while urinating. I don't see blood since three weeks of PO. I have minor discomfort most time not only sitting, but it doesn't bother me much. I think the discomfort and frequent urinating are related, and the bladder is too active. To deal with this, I am doing the same thing as you are doing, try to hold longer time, and train bladder holding more volume. It may take some more time for the wound to heal completely, then the nerves become not so sensitive. I do believe the problem caused by our years of BPH. I believe, by paying our efforts, exercise, and take more time, most remaining symptoms should be taken care of. By the way, are you doing the Kagel exercise? I am doing it every day as long as I don't forget.
