I would think that it is more likely that they will irrigate your bladder via the foley before they discontinue the catheter. They do this to make sure there are no big blood clots. If there are no big clots they then discontinue the catheter and then you have the void (urinate) once before discharge. It is important to keep drinking adequate water so that no new big clots can develop before they are voided out. You will feel a lot better with the catheter out. PS: It does not hurt coming out.

From Nate. Post HoLEP report. I’m sharing this in the hope it will encourage someone. I had a Urolift procedure 2 1/2 years ago. It was almost from the start a total failure. I would never recommend this procedure to anyone. More than likely you will end up inflammation and fibrosis from the implants. Thankfully Dr Miller was willing to take me on and removed the implants with the prostate tissue with the HoLEP procedure. I’m three days post-op. On day two my catheter was removed. I did urinate on demand. Had pressure with no pain and no gross blood. However I could not hold my urine. I was reassured that this would correct itself going forward. I had a 2 1/2 hour car ride back home to Alabama from Nashville. What was most effective and I want to recommend is i used a condom-catheter device with leg bag. Works just like a condom but had a tube attached to leg back to catch the urine. When it filled we simply pulled over to the side of the road and emptied it. I will gladly share where to purchase on line if anyone’s interested. Fast forward to this morning of post-op day three. Awoke after 4 hours sleep. Very inflated bladder. Depends make diaper was totally dry. Went to bathroom and urinated a stream about the size of a nickel. Totally emptied my bladder in 3 seconds flat. Not an exaggeration. Totally elated. Back to bed for three hours awoke in control of urine flow. I feel 18 years old again. Dr Miller rolled back my urology clock to 1973. Ha. Hope this helps someone

Hi Nate, this is Ben, I saw you had surgery on 1/18, and I did in same place on 1/19. How do you feel now? Everything to me has been OK except the bleeding, first two days was just like you, color like Hawaiian Punch, but since this afternoon it gets worse, the color become darker, which mean more blood lose. since we have similar situation, want to share the experience with you. Also I wase communicated with Phiip, hope he can see my messages too.

Ben: Be sure not to over due the activity (ie: heavy lifting, etc); and none of the forbidden acitivities such as bike riding; and do not take anything that will thin your blood (ie: NSAIDs, Aspirin, Fish oil, etc.).!!! More concentrated color might indicate more bleeding, but darker usually indicates older blood and insufficient water intake. Encouraging fluid intake is very important. If the problem persists despite increasing fluid intake and doing all the right things then you might report it to the doctor. But, personally I think you will be fine. Philip

Hi Philip, in order to be recognized, I put my name Ben here. I had surgery on 1/19 with Dr. Miller, but I guess her student did the surgery. Everything seemed fine, except the bleeding. In first two days, see some blood in urine I think it's normal, but since this afternoon, I drank a lot of water and voiding frequent, after some clots and debris came out with urine, the new urine color became darker and darker, looks more bleeding. I don't know what caused that, got little nervours, I may have to contact Dr. Miller tomorrow if this consists. How do you think? is that normal?

As I mentioned before I think you will do fine as long as you are not doing all the wrong things that I mentioned above. But it is critical that you encourage fluids --- even overnight --- even if you have to get up a few times during the night!! You need to keep you bladder flushed out if there is any bleeding because if the blood settles and clots that can cause a blockage if they get too big. See how it goes overnight and then bounce it off the doctor in the morning if you are concerned.

Ben. This is day 4 post procedure. I can hold my urine and the urinating itself is better than I imagined it would be. I do have slow dripping for a short period after. But it resolves and I do urinate if I laugh or cough but I am wearing the men’s depends. Haven’t seen many clots and the color is rather clear. Of course my wife, retired inner city school teacher, is making sure I drink copious amounts of fluids. I was a bit skeptical given the size of my prostate and the fact that I had Urolift 2 1/2 years ago (I don’t recommend) which had to be cut out along with fibrosis and my surgery was about 2 1/2 hours long. But I have responded well so far. Credit to the good Lord and Dr Miller. We are truly amazed by her and her compassion and her entire staff at Vanderbilt Urology. First class in every respect.

Ben I was concerned immediately after the procedure and called the urology center but I thought I was obstructed. I wasn’t. But they did bring me back. Dr Miller told my wife that it would 13 weeks for all the tissue to heal. Sounds like you are only 4 to 5 weeks in. I’m a novice at this but as long as I can urinate, I’m just gonna keep pouring down the fluids. Look it up online and avoid foods ( along with the meds Philip mentioned above ) that promote bleeding. The primary postop nurse ( I won’t mention her name even though she’s amazing) told me she has seen 12 men a day for years and they all eventually had all issues resolve. Telling me that pretty much took care of my anxiety.

Ben here, to Philip and Nate.
I HAD THE SURGERY ON 1/19, AND RETURNED TO HOME ON 1/21.
I was in panic yesterday afternoon. By around 3 PM, I was fine and feel everything was improving, the urine blood color was getting lighter compare to previous days. I still drink a lot of water. But suddenly, my urine blood got much more concentrated. I then drink more water, but more blood was shown every time. I wrote to Philip that time. I remember that I saw some dark red debris little before 3 pm, and more debris (bunch of) after 3 pm, the worst was after second time debris coming out, around 7 pm. Then third time debris, less amount. After that, my urine bleeding started reducing down I was released by midnight. I kept drinking water and got up during the night many times. This morning I found my urine color was much much lighter. by the time I am writing to you guys now, the color is little pinkish, compare to yesterday, I think it's only about 1 to 2 % concentration of yesterday afternoon's. what caused that? I guess, since I drank lot of water, it flushed bladder and prostate fossa constantly, it might lift the new scab from the wounds make it bleeding. Anyway, I am so glad I am through the fearing. Hope by tomorrow my urine color will be cleared up. I will start sharing my experience tomorrow.
By the way, how many follow up we have for post operation? I have an appointment with PA on 2/28, what we will do for that appointment? I am wondering if I can do what I need to here locally and send the information to Nashville, since I live in New Jersey, and try to save the trip for some simple test, but definitely will go for some major evaluation follow up. Thanks.
Over all, my trip to Vanderbilt was very good, with Dr. Miller, the facility, and entire team.

Ben: Glad to hear you sense you are doing better. I thought it was likely that you would improve with time. The post-op evaluation is just wrap things up and make sure you are well and without difficulties or concerns. They also do a flow rate study to establish a baseline for future reference and also to verify that they have achieved their goal. You could easily miss the follow up --- but it would be in your favor to attend just in case in the future you were to need some sort of assistance. It is a way to say thank you to the doctor which might pay off down the road. If it is just a meeting with a PA then perhaps contact him/her and describe your progress and see if they think you really need to show up.
