Don't worry about the dates.
Doctors base dates on your LMP. Why? Because almost all women know EXACTLY when that was. Ultrasound dating, despite what they claim, is NOT accurate to the day. Nothing is, it's all averaged. Everyone develops differently.
When did your last period begin, the ending date is irrelevant. I'm assuming around 31-10-12. You likely ovulated between 10-11-12 to 15-11-12. You could have conceived around then, within a few days.
40 weeks (normal gestation) would be August 7. We're close.

November to September is 10 months. Showing 21 weeks now would bring you to a LMP around 1 December which would match the September due date.
It appears as though your doctor feels you conceived a month after your last period.
When did you find out you were pregnant and how? Did you take a test on your own?
