The hymen already been broken from a tampon or masturbation - that's why moms and grandmas would say "Tampons are for bad girls" they meant that only girls with broken hymens could use them! It is that OLD thought of IF your hymen is torn or gone then you have HAD to have sex! And IF you question is "Can a doctor know how it was torn?" the answer is NO! IF you need to be a virgin and be tested, then tell your doctor that it happened with a much bigger tampon than you needed!

Some women don't bleed after having sex because they might have broken their hymen doing something else such as sports. Some women are even born without a hymen, so don't break out. Like babmi27 said, it can also depend on your partner.
