I have had pain in my testicals and penis for a long time. When I urinate I get flow that seems blocked because it comes out in two steams. Also, my both testicals seem to be enlarged. I have been to doctor and he gave me prostate exam and put me on antibiotics. I have been using them for 3 weeks now, but they don’t seem to help. What can actually help me with this quite embarrassing problem?

Your symptoms sound like you have prostatitis and secondary infection of testicles. Men with prostatitis (or testical infection) syndromes are common patients in the urologic office and it is nothing to be embarrassed about. Your doctor needs to screen you with urine analysis, urine culture, and urine cytology. If he finds no bacterial pathogen then he should do a special urine analysis with three samples. The three part urine analysis shows a high number of white cells on one of the three samples. Maybe this is a case with you and your testical infection. In almost 80% of cases of urine analysis the most common organism found is E. coli. If the cause is bacterial, usual complaints are genitourinary pain, dysuria or painful urination, clear urethral discharge, recurrent UTI, and sexual dysfunction. Treatment with Bactrim or one of the Fluoroquinolones that lasts 4-6 weeks is successful 30-50% of the time.
