I don't think it's an STD. It's too soon for any signs to appear. More than likely there was some trauma to his penis from having sex. Take it easy for few days. It is also possible that he injured, fractured, his penis. There can be some swelling and bruising. If it continues he should see a urologist.
There is always a chance of pregnancy. After he ejaculated the first time, did he urinate before he went back in? If not chances are even greater.
