I am male 36 yrs old. For the last 4 months I have been having pain in the upper portion of the right testicle. The initial one caused severe pain in the lower abdomen and the tingling pain in the right testicle. The pain usually lasts for about a week and then disappears. I do not see any swelling of the testicles. On examination in end of Dec the physician measured 3 cm (left) and 3.5 cm (right). He indicated the spermatic cord was bigger on the right than the left. He asked me to monitor and report back if it got worse. The pain started in october (3rd week), nov (3rd week), dec (3rd week) and Jan (1st week), feb (its just starting). I never had this pain before. I do not have pain or burning sensation when urinating or when intercourse. On a scale of 1-10 the pain is around 4. I sleep on my stomach (have been doing this for about 10 years) and occasionally toss and turn in the bed.
Any feedback on this situation will help.

It's possible that this is Epididymitis, a minor infection of the spermatacord or vas deferense, or Epididymis (on top of the testicle).
Your doctor should know this. It may fade, or re-occur, and cycle a bit, but he should know this is a possibility.
It's possibe that you're agravating this with frequent intercourse... or perhaps by masturbation.... or by handling...or impacting your wife's body, etc.
Also, you might consider wearing a jock strap or supporter when sleeping, to see if that improves things.
Another possibility is a slight herpes infection of the scrotum, that is being referred to the testicle and adomen. If you are eating a lot of citrus or vitamin C, or nuts, or something that you are allergic to, it might trigger this monthly outbreak.
If this is in time with your wife's monthly cycle, I might suspect that you're reacting to her hormones, but that woud be a stretch.

Thanks. I do not have any swelling of the testicle nor it is red in color. No discharge or pain during urination. I am afraid to go and get any tests for fear...Can Epididymitis go away without treatment ?

Yes, Epididymitis can fade away without treatment, but that doesn't guarentee that it won't return.
You really shouldn't worry about going to see a doctor about this. Some antibiotics, and if it is Epididymitis, it can be cured up.

Bro same situation here, then the doctor found out it is only just a varicose veins then he just gave me antibiotics then it dissapear, that was 3 yrs ago, then now i am having same pain again dunno why?

Two reasons a doctor would prescribe antibiotics is for a bacterial infection, either in the epididymis (epididymitis) or in the prostate (prostatitis).

Recently I experienced a similar problem. It was aching/tingling feeling in the upper right part of my testicle. It did not hurt badly, but was very distracting and embarassing to talk about. People I talked to would chalk it up to anxiety and this made me irritable and it was very difficult to focus my attention. I browsed constantly on the internet for information and looked into all the "usual" conditions and was sent home answerless by my local clinic. I spent four straight days meditating constantly on the possible cause with no progress. Then one day when I was browsing the internet for information I had a eureka moment! I stood up and looked at my seat. It had a soft arc that resembles a smile. For some reason this gave me an idea and looking down at my feet, I relaxed my leg and twisted my right foot inward. My thigh muscle, to my surprise, contracted showed no resistanc. To my surprise I was able to twist my foot around until my right foot was completely facing behind me with no pain. I similarly tested my left leg the same way and found it wouldn't turn more than 100 degrees without stiffening, not half as far. I realised my chair must have been causing my leg to rest in an odd position which must have affected bloodflow in my leg by causing my right foot to twist inward. After deciding this I immediately changed my sitting habits and began massage therapy on the inside of my thigh. I dont let my foot twist inward anymore while I sit or walk and I began to feel subtle creaking and tingling sensations indicating returning bloodflow in my groin within an hour. By the next day my testicle had dropped and the swelling and pain had cut in half. Further revised research has led me to discover muscle misallignment in the thigh is a common problem among people with weak glutes and overcompensating thighs and it can reduce circulation in the leg and groin. After a few days bed rest making sure to sleep on my back with my legs spread and feet angled outward so my overstressed inner leg muscles could rest I began doing stretches and footwork exercises by turning my foot outward and pivoting. Now I am strengthening my glutes and I notice my ejaculation and erections have returned almost to normal marking improved bloodflow. I hope this helps somebody out there as I realise the stress and anxiety can break you down. The entire scope of this event for me was two weeks to this day and I am continuing my research and therapy. I know no one has many answers regarding this issue so I want to share my story for those of you who are being patronised by your friends and doctors. This worked for me. I believe the causes were my seat, weak legs, and day long sitting habits. Please understand these symptoms are signs of serious medical conditions and only in cases like mine where all other possibilities have been ruled out by a medical professional should you accept this advice. -23 year old native american male, i have no prior health conditions but i did break my leg as a child. dont give up!
