Hello all, I have a quick question on basal lantus. I have had diabetes type 2 for 10 years now and I was recently put on Lantus and I'm uncertain about my dosage. More so about when is the best time to take it and should it be taken once a day or should it be taken twice a day. I've been out on the forums and different posts are telling me different things. Can anyone add further to this? I'm really get some highs for fasting BG and I'm wondering if I need to split up my dose. I'm taking the whole thing in the morning. Thanks.

Hi, If you're getting high fasting BG, you might want to take your lantus at night before you go to bed. I know that lantus is supposed to last 24 hours but I really think it has to do with your metabolism so it may not stretch to 24 fully. Some people take lantus once a day and that works for them. Others have found this didn't work and experimented with it by splitting it into two doses and that works for some people. I say try lantus once daily but take it in the evening before you go to bed. Do this for a week or so and check you BG and see if it works. You might want to try splitting your dose first as that is easily enough to do without disturbing your schedule. Talk with your diabetes manager to figure out how to take lantus at night when you are taking it during the day (not sure how that works). I started out with a split dose because that's what my doctor suggested and that has worked fine for me. I've never done it once daily. It's strange that different doctors tell you to do different things and don't tell you what all your options are.
