I have this exact problem and my dr said the same thing. Pressure on a cranial nerve. I wake up in the middle of the night with excruciating pain on the side I'm laying on. It's my ear and all around my ear and head. I have to roll to the other side and it takes about 20 - 30 minutes to go away. I also believe it has something to do with hormones. Sometimes that side of my head will ache the next day.

I have read that it's due to low progesterone, and that makes complete sense to me. I get the same terrible pain but ONLY right before or during my period when my progesterone level is really low, and when I slather progesterone cream on (usually after the flow has subsided), the pain does not occur. So I highly recommend you get your progesterone levels tested at the doctor to see what he or she can do.

Ask your ENT about Chondrodermatitis Nodularis. Th is was my diagnosis. I had a biopsy done ut there was not issue. Just a lot of pain that disrupts my sleep.

i've had the pain, on both ears, for past 20+ years. i'm currently 73 years old. i have gone to ear/nose/throat spec., with no answers. Doctor has done MRI of head, with no answers. dentist suggested i might be causing it because i clench my teeth while sleeping. i have a guard ordered for teeth. will see if that might work. i am able to sleep on my left side now (could not sleep on either ear for a number of years), the right one is just really too bad for me to lay on. on my left side, i have to sleep on down pillow, then i punch a hole in it where my ear will go, and put another small pillow underneath my pillow on one side so it raises the side somewhat, thereby putting the pressure mostly on upper part of my head. i do have bad allergies, but my allergist is also not able to help me. i am generally in good health. nothing to do with hormones or pregnancy. as long as i do the pillow just right, i can sleep pain-free. should i wake up with the severe pain (i agree it's very severe), i have experimented and found that if i can bear to put my fingers on the area that is painful (very carefully) and start rubbing in circles, very lightly at first and then increasing pressure, it will be only few minutes and the pain will be gone. the area i usually rub is outside of ear, at the front of it. while rubbing i hear noises like paper is crinkling. once the pain is gone, the paper crinkling noise stops. go figure. also found that if i get up, or sit up straight, it will go away by itself in about 15 to 30 min. am thinking of buying a pillow with hole in middle.

I suffer with what not many Doctors Know much about " Exploding head syndrome" yes seriously there is such a thing. It started many years ago with the symptoms just like i have been hearing on this thread, It only happens in the first hour of sleep, no treatment for it but like all of you in this thread i look to see if i can alleviate my ear pain. What i will try is the vitamin D supplement with calcium

If you have hashimotos which I do as well taking fluoride is very bad for hashimotos. It suppresses the thyroid even more. Please look up hashimotos and negative effects of fluoride. They say to not even drink fluoridated water. Also no toothpaste with fluoride. But back to the other information you gave I'm so glad you shared I will see if that could be the cause of my ear pain . Finding the cause is always the best way to treat thank you so much for sharing!

I'm having this issue for a couple of days on one side. I'm thinking it is starting in my neck or jaw. I do find that it feels better if I lay my head on a heating pad on the side that hurts.
