A man can develop a varicose vein in the scrotum or testicle area very easily, especially on the left side. The reason is the actual construction of the vein, which allows the back-up and pooling of blood.
It is probably nothing that he has done or injured. It sometimes happens without notice.
He indicates he is sometimes in pain. This might indicate that he needs to see a surgeon about making some adjustments, but many men can handle the pain with more support in his briefs.
If you are trying to become pregnant, you may want to make an apointment soon. Most men are able to become fathers with a varicose vein present.
His work on a ship is probably not affected, as he can wear a trust or supporter for relief.
There is no medicine for this condition.
For more information, make a joint appointment with his doctor, if he won't go alone. After all, a decision to make a baby is a joint decision.
Good luck.
