Technically, insertion of a penis any distance into a vagina and you're not a virgin anymore.
There is also no way to know if someone is a virgin. Even doctors can not tell, unless you just recently had sex. Not all women have intact hymens at birth and it can tear from physical activities.
Bleeding does not indicate virginity either. Not all women bleed their first time. Some women bleed after the first time.
