He could have torn ligaments or tissues. They are very common after such an accident.
You often don't feel "everything" after an accident. Your adrenalin is pumped up and it masks pain and injuries.
Have you both been seen by a doctor? It would be a good idea to go back. While it may not be serious, if there are any long term problems reporting them now may help if you end up seeking damages in the future.

There are lots of ligaments and tendons, all sorts of connective tissues. Did he hit anything, like an armrest? It could be from the seat belt too. Possibly a hematoma, bruise under the skin.
It may even be pain from something internally that "refers" to that location.
