This has been a bit of mystery for nearly 25 years.  I am a 38 year old male with a history of good health.  I have an issue with severe and I do mean severe pain in my lower back (feels like kidneys) when I need to urinate and I have a full stomach.  I've learned to live with it by making sure that if I've just eaten (within the last 30 - 45 minutes) and I need to urinate, I don't delay and I'm fine.  However, If I attempt to "hold" my urine and don't head to the restroom, I will be nearly incapacited with pain within about 15 minutes.  I've experienced kidney stones many years ago and I would put the pain in about the same threshold.  From the time the pain begins (which feels like a "tightening back in my kidneys", it only takes about 3-5 minutes for the pain to become unbearable.  Even after urinating it takes about 5-7 minutes of severe pain before it begins to subside and roughly 15 minutes to be back to near normal.  During this time the pain and tighteness will work it's way around my sides and cause my stomach to tighten.  I will begin sweating profusely, and feel nausea but I think this is in large part due to the pain.  There is not any pain associated with the actual urination process, it is all located above my waist.  Any help or thoughts would be appreciated.

This could be more stones, detection and treatment have improved in the last decade.  Although you may need 2 or 3 different types of scan to have a really good chance of ruling out a stone.

Incidently this isn't something I've heard of so please share the outcome! :)
