I had one external, thrombosed hemorrhoid removed that, prior to removal, had caused me severe pain for over a week and with two more weeks with moderately high pain- especially when passing BMs.  This was my third severe bout with this issue and the other two times lasted about a week each.   At first the colorectal surgeon stated that we should try conservative measures-- sitz baths, Motrin every 4-6 hrs (to try to shrink the thrombosed hemorrhoid) and high fiber diet.  But after 3 weeks, the dr said let's go ahead.  She said that only bc I was Already in a lot of pain that the post procedure pain would be tolerable for me and even an improvement.  But she did warn me it would be very painful for 4 days and my recovery would be about 3 weeks to feel more "normal".  Yes, the pain was terrible the first day., I cried after the lidocaine wore off and Until the pain meds kicked in.  If you can get them ahead of time, do that.  I was Ordered Tramadol 50mg every 4 hrs and to alternate Tylenol and Motrin every 4-6 hrs (so I would take Tylenol or Motrin every 2-3 hrs).  The next morning. I was doped up on the Tramadol but pain was much better--except-- side effects kicked in and I had A headache and nausea and vomiting from the Tramadol.  I had to take a zofran(which i had at home) and I slept All day and had to stop the Tramadol.  I continued With the Tylenol and Motrin once I could Keep stuff down in the evening.  The pain was already better from when I was Crying the first day.  Also I had  A BM that wasn't as bad as I thought-- but still very painful.  Like everyone else-- I found The tub to be my best friend.  For me-- I feel this suffering will be worth the long term quality of life.    My hemorrhoid issue wasnt as bad as some I've read about.  So Prior to surgery-be prepared to take 2 weeks off work(more if you have extensive issues), invest in a sitz bath, get gauze pads amd topical lidocaine cream.  Ask your surgeon if you can pick up your pain med pre-procedure, and if not, be ready to send someone to get it ASAP post procedure.  I'm 4th day post procedure and not very comfortable, not very happy, not eating well, not pooping well.  But I'm very optimistic that this will soon be a distant memory.  People have said it's worse than childbirth.  I'd say it's about the same or worse because the recovery time is longer with the amount of pain to have.  It's definitely not for everyone.

Hi there ... sounds terrible. Do you feel any better now? I'll also need to have hermorrhoidectomy but do not have that much issues, nor thrombosis, but I have a lot of disturbances during BMs. When did you notice the hermorrhoidectomy is inevitable, like when the pain kicked in or even earlier? Sitzbath is actually only to get temporary relief, but I have been told that the only perma solution is the hemorrhoidectomy and nobody said the recovery went smooth :(
