I've had this problem several times over the past few years.. turns out I have chronic bladder infections. If you have a low-grade bladder infection, this can cause your scrotum and often your penis to constrict. You may not be aware you have it, but if you notice that your urine stream feels restricted, or you can't seem to completely empty your bladder, you may have it. Some men have chronic bladder infections that go away when treated with antibiotics but often return within a few weeks. Since you don't want to take antibiotics several times a year, it is better to take other approaches to treat a (possible) bladder infection. What has worked for me: 1. Take an anti-inflammatory (I like ibuprofen); 2. Take cranberry pills; 3. Drink lots of water; 4. While following this regimen, hold your urine longer than usual, and when you do urinate empty your bladder completely. Other things that I have noticed help to relax the scrotum: taking magnesium glycinate every night before bed; wearing loose underwear; staying warm. I just went through 4 mos of a nicely loose scrotum, but things just tightened up again, so I'm back on this plan. Hope it helps someone.
