Hurts when I poop, and sometimes it bleeds. I don't think it's my period blood. What else?

Hi I have been having this problem for a while, whenever I go to the bathroom it's hard to poop and it hurts me plus sometimes when I wipe I see blood, I'm not sure what to do.
